The income of ex-servicemen/widow from all sources should be less than Rs 25,000/-

In case Martyr is married -
Widow - Rs. 1,00,000/-
Rs. 50,000/- each to Father & Mother (Additional grant of Rs 2 lacs, if funds are available)
(b)
In case Martyr is unmarried -
Father - 1,00,000/ -Mother - 1,00,000/-
Application in triplicate as per annexure I to be submitted to Director Sainik Welfare, Punjab in triplicate.
Affidavit of each beneficiary in original duly attested by Executive Magistrate along with two attested photo stat copies.
Three photo stat attested copies of battle casualty/accident report.
In case father/mother is already dead, original copy of death certificate issued by Registrar (Birth & Death) along with ,two attested photo stat copies.
In case of dependent children, their birth certificates/school admission certificate/part-II order regarding birth casualty issued by Army authority.
Where endorsement of home address of Martyr or NOK/information regarding NOK is not mentioned In Battle Casualty /accident report by the Army/Para Military force, a certificate to be obtained regarding permanent home address.
