FINANCIAL ASSISTANCE TOWARDS MEDICAL GRANT BY KSB OUT OF RMDF
  Benefits:-
 

  • Medical treatment for non ECHS Members :-

    (i)   depending on nature and gravity of ailment

    Rs 30,000/- (Maximum)
    		•
      Eligibility Conditions:-
     
  • All ESM/their widows who take treatment in Civil (Govt/Private) Hospital on the Recommendation of MH.
    		•
     
  • All ESM/their widows who are not in receipt of fixed medical allowance.
    		•
     
  • All ESM/widows/dependent in receipt of Fixed Medical allowance of Rs 100/- p.m. But had to spend     more than Rs 2400/- on the present treatment for which the assistance is sought.
    •  
     
  • Cases of officers who have incurred expenditure of over Rs. 50,000/- on treatment and have not been     assisted from any other source.
    		•
     
  • All cases of PBORs/their widows be considered
    		•
     
  • All cases involving accidents.
    		•
      Documents Required:-
     
  • Application from the individual. (Specimen att as Annx I)
    •  
  • Copy of the ESM/Widow/Dependent Identity Card.
    •  
  • An Attested Photocopy of the Ration Card.
    		•
     
  • Detailed Financial Condition report.
    		•
     
  • An attested copy of the Discharge Certificate.
    		•
     
  • Certificate from the Zila Sainik Board that the applicant has not been provided any financial assistance     from the state Govt or any other source.
    		•
     
  • Whether the Ex-Servicemen was a member of AGI/AFGI medical insurance scheme. If yes, whether     he/she has been provided financial Assistance from them.
    		•
     
  • Any reference from Military Hospital/reason as to why the ex-servicemen/patient did/could not visit the     MH for treatment.
    		•
     
  • Discharge Summary / Diagnostic sheet from the hospital.
    		•
     
  • All medical bills (in original) duly countersigned by the attending doctor/competent medical authority.
    •
