  • Application from the Widow (Specimen att as Annx)
    		•
     
  • Photocopy of Service Particulars / Discharge Book of the deceased soldier.
    		•
     
  • Photocopy of Pension Book.
    		•
     
  • Postal Order of Rs 5/- in favour of Secretary Flag Day Fund.
    		•
     
  • Three Photographs of the Widow duly attested by DSWO.
    •  
  • Photocopy of Identity Card and Ration Card.
